Hertfordshire Partnership University NHS Foundation Trust (HPFT) is seeking a passionate and dedicated Community Mental Health Nurse (Band 6) to join the Adult Community Mental Health Services Team in North Hertfordshire / Stevenage.

Main Duties and Responsibilities

  • Manage a defined caseload of adults with complex mental health and social care needs
  • Assess, plan, implement, and evaluate individualised care programmes
  • Act as Care Coordinator under the Care Programme Approach (CPA)
  • Maintain accurate and timely electronic patient records
  • Administer and monitor prescribed medication in line with Trust policies
  • Provide liaison across inpatient, primary care, and community services
  • Support carers and relatives in emotionally demanding situations
  • Participate in nurse-led clinics (e.g. Clozapine)
  • Mentor and support student and learner nurses
  • Contribute to safeguarding, risk management, audits, and service improvement

Essential Qualifications

  • Registered Mental Health Nurse (RMN)
  • Diploma or BSc (Hons) in Mental Health Nursing
  • ENB 998 / Preceptorship & Mentorship

Desirable:

  • Specialist Community Nursing qualification
  • Psychosocial intervention training

Experience Required

  • Post-registration experience at Band 5
  • Experience in acute mental health inpatient or community services
  • Crisis and emotionally demanding environments
  • Safeguarding knowledge and practice
  • Multi-disciplinary and multi-ethnic community experience
  • Evidence-based care delivery
