Overview
Hertfordshire Partnership University NHS Foundation Trust (HPFT) is seeking a passionate and dedicated Community Mental Health Nurse (Band 6) to join the Adult Community Mental Health Services Team in North Hertfordshire / Stevenage.

Main Duties and Responsibilities
- Manage a defined caseload of adults with complex mental health and social care needs
- Assess, plan, implement, and evaluate individualised care programmes
- Act as Care Coordinator under the Care Programme Approach (CPA)
- Maintain accurate and timely electronic patient records
- Administer and monitor prescribed medication in line with Trust policies
- Provide liaison across inpatient, primary care, and community services
- Support carers and relatives in emotionally demanding situations
- Participate in nurse-led clinics (e.g. Clozapine)
- Mentor and support student and learner nurses
- Contribute to safeguarding, risk management, audits, and service improvement

Essential Qualifications
- Registered Mental Health Nurse (RMN)
- Diploma or BSc (Hons) in Mental Health Nursing
- ENB 998 / Preceptorship & Mentorship
Desirable:
- Specialist Community Nursing qualification
- Psychosocial intervention training
Experience Required
- Post-registration experience at Band 5
- Experience in acute mental health inpatient or community services
- Crisis and emotionally demanding environments
- Safeguarding knowledge and practice
- Multi-disciplinary and multi-ethnic community experience
- Evidence-based care delivery
