Social Worker positions are available across various locations:

  • Broad Oak Hospital
  • Broad Green & Norris Green Hub
  • Baird House
  • Moss House

 Role

As a Social Worker in our City-Wide Mental Health Assessment and Care Delivery team, you will:

  • Deliver interventions to adults with a range of mental health-related care and support needs
  • Use a strengths-based approach in Care Act assessments, reviews, s117 aftercare planning, and s42 safeguarding enquiries
  • Co-produce support plans that promote inclusivity, collaboration, and innovation
  • Work in a multi-agency environment alongside co-located health partners across Liverpool
