Salary: £24,937 – £26,598 per annum (pro rata)

Overview
Cornwall Partnership NHS Foundation Trust is transforming Community and Palliative End of Life Care across the county. Our goal: to ensure people nearing the end of life can experience a peaceful, dignified death in the place of their choosing.
We’re reducing unnecessary hospital admissions and delivering personalised care closer to home through initiatives such as:
- 24/7 Rapid Community Response and Care Service
- A dedicated End of Life Care phone line
- Expanded voluntary sector partnerships and our Hospital@Home virtual service

Main Duties
- Assist in delivering clinical care planned by registered healthcare professionals
- Provide day-to-day care for patients, respecting their needs, wishes, rights, and beliefs
- Work within the 24/7 Telephone Response Service for Palliative & End of Life Care
- Support admission avoidance services to keep patients safe at home
- Record referral information and direct callers to the right services with clinical team support
- Contribute to resource management within the service
Person Specification
Essential
- NVQ 3 in Direct Care (or working towards)
- Previous clinical experience in a healthcare setting
- Knowledge/skills in clinical procedures (e.g. venepuncture, aseptic technique, basic observations)
- Ability to organise and carry out designated workload competently
