Salary: £54,320 – £60,981 per annum (inclusive of HCAS)

About the Role

South London and Maudsley NHS Foundation Trust (SLaM) is seeking a skilled and autonomous Primary Care Network Senior Mental Health Practitioner to join our Lewisham Community Services team.

This role is part of a wider initiative to enhance mental health provision in primary care, following a successful pilot. The postholder will provide early intervention, assessments, and tailored support to patients, while facilitating seamless transitions between primary and secondary care.

Key Responsibilities

  • Conduct assessments, care planning, and interventions for patients with mental health needs.
  • Support service users and their families with education, advice, and self-management strategies.
  • Facilitate safe transitions between primary and secondary care.
  • Provide expert consultation and support to GPs and primary care staff.
  • Deliver peer support to other Primary Care Mental Health Practitioners.
  • Promote quality improvement initiatives across PCNs.

Person Specification (Essential)

  • Registered mental health professional (RMN or equivalent).
  • Evidence of post-qualification training and CPD.
  • Strong understanding of the relationship between primary and secondary care.
  • Skilled in clinical interventions (e.g., depression, anxiety, family work).
  • Excellent written and verbal communication skills.
  • Experience in community mental health and working with patients with severe, ongoing mental health needs.

Desirable

  • Non-medical prescribing qualification.
  • Experience working in primary care settings.
