About the Role
Liverpool City Council is recruiting an Adult Social Worker to join the Multi-Disciplinary Team (MDT) within Adult Social Care. This role offers an opportunity to make a meaningful difference by delivering high-quality, person-centred services to adults aged 18 and over.
Adult Access is the front door to Adult Social Care and Health, ensuring clear direction for enquiries, demand management, and accountability. The service operates across three key functions: Wellbeing Advisors, the Multi-Disciplinary Team, and Safeguarding Triage. All teams work together using a strengths-based approach to promote independence and reduce long-term care needs.
Responsibilities
- Assess and provide reablement or short-term support to promote independence
- Complete robust assessments to support smooth transitions to Locality Teams when long-term intervention is required
- Undertake Needs Assessments, Risk Assessments, Mental Capacity and Best Interest Assessments, and Reviews
- Manage a diverse caseload of adults aged 18+
- Commission services including Domiciliary Care, Day Centre placements, Respite Care, and complex support packages
- Participate in daily MDT huddles to discuss and manage complex cases
- Ensure compliance with statutory duties, council policies, and national guidance
