Overview
Hackney Council is recruiting an experienced Adult Social Worker to join the Hospital Discharge Team. This role focuses on ensuring safe and timely hospital discharges by assessing individuals’ needs and arranging the appropriate care and support in the community.
You will work within a multidisciplinary hospital environment, collaborating with NHS staff, discharge coordinators, families, and community services to ensure effective discharge planning.
Key Responsibilities
- Undertake strengths-based assessments to support safe hospital discharge
- Develop and implement person-centred care and support plans
- Work closely with NHS colleagues and community services
- Facilitate timely discharges with appropriate care packages in place
- Undertake safeguarding enquiries where required
- Apply legislation including the Care Act 2014 and Mental Capacity Act
- Maintain accurate case records and statutory documentation
Requirements
- Qualified Social Worker with Social Work England registration
- Experience in adult social care, ideally within a hospital discharge or acute setting
- Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures
- Ability to work effectively in a fast-paced multidisciplinary environment
- Strong assessment, communication, and decision-making skills
