Westminster City Council is recruiting an experienced A&E Hospital Social Worker to work within the Accident & Emergency department at St Mary’s Hospital. This role focuses on ensuring safe and timely hospital discharges for patients.
Key Responsibilities
- Hospital discharge and triage work
- Care Act assessments and support planning
- Safeguarding enquiries (Section 42)
- Arranging care packages
- Providing professional advice to hospital staff
Requirements
- Social Work England registration
- Proven hospital discharge experience
- Strong knowledge of the Care Act and safeguarding
- Ability to work effectively in a fast-paced environment
