Overview
Join the Adult Community Mental Health Services team at Hertfordshire Partnership Foundation Trust as a Band 6 Community Mental Health Nurse. This is a great opportunity to expand your experience in community-based mental health care while being part of an Outstanding-rated NHS Trust. You’ll work as part of a supportive multidisciplinary team, helping to improve the lives of adults experiencing mental health challenges.
Main Responsibilities
- Conduct comprehensive assessments and create personalised care and support plans.
- Manage a complex caseload with guidance from senior staff.
- Participate in depot and clozapine clinics, including medication administration and monitoring.
- Perform physical health monitoring (blood pressure, weight, etc.).
- Contribute to the duty desk and first-contact assessments.
- Maintain accurate records and comply with safeguarding, risk management, and governance standards.
Requirements
Essential:
- Registered Mental Health Nurse (RMN) or BSc (Hons) in Mental Health Nursing
- Post-registration experience (Band 5 or above)
- Experience in acute or community mental health care
- Excellent communication and teamwork skills
- Strong assessment and clinical decision-making abilities
Desirable:
- ENB 812 or BSc in Specialist Community Nursing Practice (Mental Health)
- Phlebotomy skills (training available)
- Experience working in community crisis mental health settings
