Overview
Hertfordshire Partnership Foundation Trust is seeking enthusiastic Community Mental Health Nurses (Band 5) to join our Adult Community Mental Health Services in Cheshunt.
This is an excellent opportunity to develop your knowledge and experience in a community mental health setting, working closely with colleagues in a supportive, small-team environment, making a real difference in service quality.
Important Requirement: A full valid driving licence and access to a car for business use, including business insurance, is essential unless exempt under the Equality Act 2010.
Key Responsibilities
- Be part of a multi-disciplinary team, delivering high-quality care collaboratively and compassionately.
- Manage a patient caseload agreed in consultation with medical, nursing, and other professionals.
- Conduct specialist mental health assessments and deliver tailored care plans.
- Administer, store, and dispose of medication safely according to Trust policy.
- Plan and support safe patient discharges following agreed care pathways (CPA).
- Ensure care is person-centred and recovery-focused, challenging practices that may hinder high-quality care.
- Utilize research and evidence-based practice in community mental health.
- Support carers of service users, including self-help groups and respite care initiatives.
- Participate in adverse incident reporting and risk management processes.
Newly registered Band 5 nurses will be supported through a 12-month preceptorship programme to aid the transition from student to nurse.
Person Specification
Essential Qualifications & Experience
- Registered Mental Nurse (RMN) or Dip/BSc (Hons) in Mental Health Nursing.
- Band 5 post-registration experience in a relevant mental health setting, or participation in the Trust Preceptorship Scheme if newly qualified.
- Knowledge of MHA (1983), CPA, Risk Assessment, Section 25.
- Ability to assess, plan, implement, and evaluate nursing care under supervision.
- Effective multi-professional team communication and teamwork.
- Medication administration competency.
Desirable Qualifications & Experience
- ENB 998 / Preceptorship and Mentorship.
- Experience with acute mental health problems in community settings.
- Evidence of continuing professional development.
- Understanding of primary/secondary care interface.
- Experience in managing change successfully.
