Overview

HPFT is recruiting enthusiastic Band 6 Community Mental Health Nurses to join the Adult Community Mental Health Service in Welwyn Garden City. You’ll work within a supportive multi-disciplinary team, delivering person-centred care and evidence-based interventions to improve outcomes for service users.

Main Responsibilities

  • Carry and manage a clinical caseload as the named nurse.
  • Complete comprehensive personalised care plans and risk assessments.
  • Administer oral and injectable medications and monitor physical health indicators.
  • Work collaboratively with families, carers, and multi-professional teams.
  • Maintain accurate clinical records and prepare required reports.
  • Apply relevant mental health legislation and safeguarding procedures.
  • Deliver evidence-based interventions in community settings.

Person Specification (Essential)

  • RMN / Dip / BSc (Hons) in Mental Health Nursing
  • Minimum 2 years’ post-registration experience (including Band 5)
  • Experience in acute inpatient or community mental health
  • Knowledge of legislation, ethical practice, and risk management
  • Strong communication, assessment, and clinical decision-making skills
  • Ability to work autonomously and within multidisciplinary teams
