Overview
Sussex Partnership NHS Foundation Trust (SPFT) is seeking a Mental Health Liaison Practitioner to join their Crawley team. This is a permanent full-time position for Registered Mental Health Nurses, Social Workers, or Occupational Therapists.

Key Responsibilities
- Act as a link between primary and secondary care, supporting GPs and primary care teams.
- Assess individuals with complex mental health presentations and develop care plans.
- Provide advice, consultation, and shared care to primary care colleagues.
- Facilitate smooth patient journeys into secondary care when necessary.
- Deliver training, guidance, and supervision to students and other practitioners.
- Ensure all practice is evidence-based and aligns with NICE guidelines.
Who We Are Looking For
Essential Qualifications & Experience:
- Registered Mental Health Nurse, Social Worker, or Occupational Therapist
- Post-qualifying clinical experience of at least 3 years
- Experience managing caseloads and delivering mental health interventions
- Strong partnership working skills across statutory and non-statutory teams
