Salary: £40,000 – £43,000 (Employer Provided)
Overview
St Helens Borough Council is seeking an experienced Senior Social Worker to join the Adults Services team. The role is primarily based at Newton Community Hospital, supporting safe and timely hospital discharges and providing specialist support to adults, older people, carers, and families.

Key Responsibilities
- Conduct needs-led assessments for adults and carers
- Develop and coordinate effective care packages
- Facilitate safe and timely hospital discharges
- Work collaboratively within a multi-agency setting
- Apply strong knowledge of relevant legislation and care management
- Support rehabilitation processes, particularly for older people
- Engage with adults, carers, and individuals with disabilities
Essential Requirements
- CQSW, DipSW, CSS, or BA (Hons) in Social Work
- Registration with Social Work England
- Knowledge of legislation, rehabilitation, risk factors, and care management
- Experience working with older or disabled adults
- Multi-agency working experience
- Willingness to undertake essential training
- Enhanced DBS Disclosure
- Ability to attend the St Helens office regularly and travel locally
