 Overview

The Care Navigation Centre provides a single point of access for healthcare professionals who require support to keep patients at home or avoid unnecessary emergency department admissions. An exciting opportunity has arisen for a Clinical Sister, Charge Nurse, or Clinical Practitioner to join this well-established team. The successful candidate will support patient triage and ensure individuals receive appropriate care through community services.

Main Duties and Responsibilities

The successful candidate will provide triage and clinical decision support for patients requiring community healthcare services. Responsibilities include coordinating referrals to appropriate healthcare teams and working closely with multidisciplinary and multi-agency teams. The role requires maintaining strong links with both community and acute healthcare services to ensure effective patient care. The position also involves communicating with healthcare professionals and patients via telephone consultations, maintaining accurate records, and supporting hospital avoidance strategies. The service operates across a 24-hour period, seven days a week, and staff work flexibly on a rota basis.

Essential Requirements

Qualifications Applicants must be a registered Nurse or Paramedic (Level 1 or Level 2). Candidates should be educated to diploma level or demonstrate equivalent experience through post-registration clinical practice at Band 5 level. Evidence of ongoing professional development is required.

Knowledge and Experience Applicants should demonstrate knowledge of legislation related to primary care services and have experience working in multidisciplinary or multi-agency environments. Candidates must also have post-registration experience within primary or secondary healthcare settings. Strong IT skills and good telephone communication abilities are essential.

Desirable Skills

Desirable criteria include knowledge of NHS policies related to hospital avoidance and an understanding of clinical governance frameworks.
