Overview
Hertfordshire Partnership University NHS Foundation Trust (HPFT) is seeking an experienced Band 6 Social Worker to join our Community Mental Health Service for Older Adults across East Hertfordshire.
You will:
- Manage a caseload of service users experiencing or recovering from mental health difficulties.
- Complete holistic assessments, support plans, reviews, and CPA in collaboration with multidisciplinary teams.
- Work alongside Psychiatrists, Psychologists, Nurses, OTs, and Support Workers.
- Provide advice and support to service users, families, and carers, promoting choice and independence.
Main Duties
- Apply knowledge of key legislation (Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983/2007).
- Undertake assessments and care planning focused on personalisation, choice, and control.
- Work in partnership with service users, carers, and professionals.
- Promote safeguarding and support anti-discriminatory practice.
Person Specification
Essential:
- Qualified Social Worker (DipSW or equivalent), registered with Social Work England.
- Minimum 2 years’ post-qualification experience.
- Knowledge of legislation and policies relevant to mental health and older adults.
- Experience with assessments, personal budgets, and direct payments.
- Strong communication, analytical, and IT skills.
Desirable:
- AMHP (Approved Mental Health Professional) or Best Interests Assessor training/experience.
- Experience in integrated health and social care.
