Introduction
We are recruiting Social Workers to join our Adults Intermediate Care Team (AICT) and welcome applications from newly qualified to experienced Social Workers.
Salary
- NQSW – Band 8: £36,363 – £39,151 per annum
- Social Worker – Band 9: £39,151 – £42,838 per annum
Pay band and salary are dependent on experience.
- Social Workers who have not completed the Assessed and Supported Year in Employment (ASYE) will be appointed at Band 8 and progress to Band 9 upon completion.
- Experienced Social Workers will be appointed at Band 9 and are expected to supervise unregistered staff.

The Adults Intermediate Care Team brings together services supporting people through:
- Admission avoidance
- Acute hospital care
- Inpatient services
- Rehabilitation and reablement
The team works across:
- Portsmouth Community Rehab Service
- Transfer of Care Hub
- Queen Alexandra Hospital
- Inpatient units
This integrated approach improves outcomes for adults following a crisis or decline in health.
Your Role
You will work within a multidisciplinary team of Social Workers and Social Care Associate Practitioners, assessing the needs of adults and carers under the Care Act 2014.
Key responsibilities include:
- Strengths-based assessments and outcome-led interventions
- Supporting hospital discharge and admission avoidance
- Completing assessments, reviews, and person-centred support plans
- Supporting individuals with health, mental health, and social care needs
- Assessing mental capacity where required
- Managing your own caseload
- Supporting and supervising unqualified staff (Band 9 role)
The role offers variety, autonomy, and opportunities to work across different settings.
Person Specification
You must:
- Hold a recognised Social Work qualification
- Be registered with Social Work England
- Have knowledge of adult safeguarding and legislation
- Communicate effectively with diverse individuals in complex situations
- Manage risk and maintain accurate, confidential records
- Work collaboratively while advocating for individuals and carers
- Demonstrate professional curiosity and motivation
Applicants for Band 9 must demonstrate experience supervising and supporting staff.
