Overview
Join a dedicated team supporting older adults with dementia and severe mental health conditions. This role involves:
- Managing a varied caseload
- Investigating safeguarding concerns
- Conducting statutory assessments
- Commissioning and developing support plans
- Presenting reports to panels or in court
- Collaborating with partner agencies, carers, and families to build support networks
Hybrid Working: Combination of office-based work, client visits, and working from home. Travel will be required.
Requirements:
- Strong knowledge of health and social care legislation and safeguarding procedures
- Excellent communication and reflective practice skills
- Experience working with older adults is desirable
