Job description

This role is based within the Adult Social Care Review Team, responsible for carrying out reviews and reassessments across the Wirral. The team works with individuals recently discharged from hospital and those living in the community, while also supporting adult social care projects and at-risk care home and domiciliary providers.

You’ll collaborate closely with contracts & commissioning, quality improvement teams, and adopt a legally literate, outcomes-focused approach. This is a fantastic opportunity for practitioners who want to strengthen their knowledge, gain CPD, and broaden their practice.

 Responsibilities

  • Carry out reviews and assessments
  • Collaborate with internal teams to deliver quality care and support
  • Promote wellbeing and improve outcomes for adults with care needs
  • Apply legal frameworks and best practice in service delivery
  • Engage with adults in hospital and community settings

Requirements

  • Registration with Social Work England
  • Recognised social work qualification
  • Evidence of Continuous Professional Development
  • Full driving licence and access to a vehicle

Additional Information

  • Appointment subject to interview, references, enhanced DBS check, and occupational health clearance.
  • Visa sponsorship is available under the Skilled Worker route. Applicants must be registered with Social Work England.
