Overview
We are seeking compassionate and dedicated Home Care Service Providers to deliver personalized in-home care to clients with diverse needs. This is an opportunity to make a meaningful difference in the lives of individuals requiring assistance with daily activities, health monitoring, and emotional support.
Key Responsibilities:
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and mobility support
- Provide medication reminders and assist with medication administration
- Prepare nutritious meals tailored to dietary needs
- Monitor vital signs and report any changes to healthcare professionals
- Support clients with memory care needs (Alzheimer’s, dementia) using specialized techniques
- Operate assistive devices and manage safe transfers and positioning
- Maintain accurate documentation adhering to HIPAA regulations
- Perform light housekeeping tasks to ensure a safe living environment
- Accompany clients during outings or community activities as needed
- Follow safety protocols including CPR and First Aid when required
Skills & Qualifications:
- Proven experience in home & community care, assisted living, or long-term care
- Knowledge of patient observation, vital signs monitoring, and care plans
- Experience with individuals with developmental disabilities, autism, or dementia
- Ability to perform safe heavy lifting using equipment like Hoyer lifts
- Strong communication skills for interacting with clients, families, and healthcare teams
- CPR certification and First Aid training preferred
- Compassionate attitude and patience when working with vulnerable populations
