Job Description 

As this service operates 24/7, flexibility is essential. You must be able to work varied shifts, including day/evening shifts, waking nights, long days, alternate weekends, and bank holidays.

St Anne’s Community Services is seeking dedicated Mental Health Support Workers to join our 24-hour supported living services at Hartwell Road, Belle Vue Road, and Outreach in Leeds.

 

 Duties

  • Adaptable, caring, confident, and well-organised.
  • Experience in a care environment is desirable.
  • Team player in a fast-paced setting.
  • Flexibility to cover shifts and work across local services.
  • Communicate confidently with professionals and external services.
  • Calmly manage and report safeguarding or incident concerns.

Responsibilities

  • Crisis response to incidents like self-harm or substance misuse
  • Teach coping skills (mindfulness, problem-solving)
  • Promote mental wellness (stress, sleep, exercise)

Life Skills & Practical Support

  • Help with daily tasks: cooking, hygiene, cleaning
  • Assist with budgeting & financial management
  • Support medication and health appointments
  • Facilitate community engagement & activities

Person-Centred Care

  • Create and follow individualised support plans
  • Promote independence, respect and dignity
  • Work collaboratively with families and professionals

Record Keeping & Communication

  • Maintain accurate records of support and incidents
  • Write clear, concise daily notes
  • Use digital tools (e.g. Nourish, Civica, Teams, Outlook) after training

Person Specification

Essential:

  • Willing to work towards or already hold the Care Certificate

Desirable:

  • Previous care experience (formal or informal)

Additional Qualities:

  • Understand safeguarding and support safe environments
  • Able to do light domestic work (cooking, cleaning, laundry)
  • Ready to undergo further training in care
  • Strong team player and communicator
