Summary
Derbyshire Community Health Services is seeking a Support Worker Supervisor to join the Community First Team in Derby City.
This role is central to patient care—supporting hospital discharges and delivering essential short-term and reablement care. You’ll supervise support workers, ensuring effective, compassionate, and timely home care.
If you have prior caregiving experience, strong leadership skills, and a passion for helping people live independently, this could be the next step in your NHS career.
Key Responsibilities
- Supervise and support a team of support workers
- Allocate patient visits effectively and efficiently
- Act as a role model, maintaining high clinical and professional standards
- Work alongside therapists and nurses in patient care and rehabilitation
- Communicate clearly with patients and carers to maximise rehabilitation outcomes
